What factors affect training form? Does it have to do with facilities or coaches, or is it just random?

it depends only on luck and training workload

In regards to training workload, bad form means I have them on too heavy of a training regime or not enough?

training form is a probabilistic result based on the training workload you have set for each player. The probability of getting bad form is:
-50% for light workload
-5% for medium workload
-0% for heavy and very heavy workloads

bad form just means the player didn't improve as much as he ought have

How important it is to train Off the ball and Teamwork in pass game, and if anyone has an idea with skil affects more, or are all equal?

Player Action : Creativity, Off the ball, Passing, Technique, Teamwork
Opponent Player : Decisions, Positioning, Tackling, Marking

From rules: chapter 12 section Opportunities to score

Quote

Corner : Setpiece, Crossing

Freekick : Setpiece, Finishing, LongShots

Penalty : Setpiece, Finishing, Technique

So I don't think there are more. Naturally experience gives some, too

Another question "IS THERE ANY WAY TO INCREASE PROGESS RATE?"
I have observed some players (CA3/LA6) oscillate between PR3 and PR4 depending upon playing times while others (same CA3/L6) don't change.

$:-\$ I'm curious. What is the max stat that anyone has seen on the top coaches? I have had several and have never seen a stat above 20. Is 20 the highest stat or do coaching stats get higher?

For the record, coaches are certainly overpriced in comparison to the improvement they provide. Those looking at this option should consider the financial aspect of this decision.

PR is same in all games, there is no way to increase it

Is it a mistake or correct? Is it means that player with rating 9 get more training then other with 5? If it is true better way is to play in lower league , get more wins then go promoted and get many looses!
